The forms listed below need to be completed by all coaches/bench staff that will be listed on your team sheet. Complete these forms and submit with your Criminal Records Check to your team designate who in turn will submit to your division director.Team Personnel FOIP FormNB **If you have completed and handed in a criminal records check within the past 2 years you will not require a new one. If for any reason you are not sure you can contact the office @ 780-459-4052 who can check for you.
