It has been brought to the attention of the Raiders Hockey Club that those players receiving invitations to Junior camps are receiving the wrong form from the Junior teams according to Hockey Alberta.

The correct form can be found under Forms/Miscellaneous Forms from the main menu and selecting 'Conditioning Camp Permission Form'. This form must be completed and taken to your local Association for authorization.
